FORM 1 — CLIENT CDM BRIEF & PROJECT INFORMATION FORM

    CDM Project Information & Pre-Construction Information Form

The information provided will assist us in fulfilling our duties under the Construction (Design and Management) Regulations 2015 (CDM 2015), including preparation of Pre-Construction Information and coordination of health and safety matters during the design and construction process.

    3. Existing Site & Building Information

    This section assists with identifying existing hazards, constraints and pre-construction information requirements.

    This helps identify risks relating to occupants, visitors and safe separation of construction activities.


    4. Workforce & CDM Notification Information

    Under CDM 2015, certain projects must be formally notified to the Health & Safety Executive (HSE). The following information assists with determining whether notification may be required.

    Person-days means the total number of days worked by all workers combined throughout the project. If unknown, provide your best estimate.


    5. Existing Information & Available Documentation

    Please select all surveys, reports or documentation currently available for the project or site.


    6. Known Site Hazards & Constraints

    Please identify any known hazards, restrictions or unusual site conditions which designers or contractors may need to consider.

    Please provide any additional details relating to access restrictions, neighbouring properties, buried services, environmental concerns or known structural issues. If none, enter N/A.


    7. Existing Project Appointments

    Please provide details of any consultants, designers or contractors currently appointed to the project. If not applicable, enter N/A.


    8. Future Maintenance & Access Requirements

    CDM Regulations require consideration of how the completed structure will be safely maintained, accessed, cleaned and repaired throughout its lifespan.

    For example: roof access systems, façade cleaning, plant maintenance, specialist access equipment or confined maintenance areas. If none known, enter N/A.


    9. Document Uploads
